Objective: To describe the diagnosis of pregnancy in an unicornuate noncommunicating rudimentary uterus, its obstetrical complications and the follow-up of fetal well-being through ultrasonography. Case description: Second-pregnancy woman with previous abortion, 33-year-old, ecography for 27 weeks, it was suspected both noncommunicating rudimentary horn pregnancy and fetal growth restriction. Patient developed hypertension, which was controlled with Alfametildopa. Two weeks later, she presented peritonism with lowering hemoglobin and, to ultrasound of free liquid in cavity it was pointed out pregnancy interruption for uterine rupture suspicion. Chirurgical finding confirmed the diagnosis of pregnancy in noncommunicating rudimentary horn, where it was carried out hemi-hysterectomy and the pathological study with placenta increta found. It was extracted an alive fetus, female, 480g of weight, with 29 weeks with clinical signals of growth restriction, which died 10 days later. Conclusion: This is a rare clinical case, where the suspicion came through ultrasonography, being resonance complimentary and chirurgical finding confirmed diagnosis. Conservative conduction was carried out due to possible cautious ultrasonographical following-up.
