Abstract

The Herlyn-Werner-Wunderlich syndrome, also called Ohvira syndrome, is a rare mullerian malformation characterized by uterus didelphys with obstruction of hemivagina by a vaginal septum, associated with ipsilateral renal anomaly. The clinical picture is characterized by dysmenorrhea and pelvic pain that arises after menarche and is often associated with the presence of vaginal or pelvic mass due to accumulation of blood in the blocked hemivagina. The clinical diagnosis of Ohvira syndrome is difficult and it is necessary imaging studies, among which ultrasound and MRI play a decisive role to confirm the uterine malformation and the associated renal anomaly and treatment planning. Treatment usually involves resection of the vaginal septum to unblocking the hemivagina, symptom relief and preservation of reproductive capacity.
